Homeschool Evaluation Forms

Please select and print the appropriate Pre-Evaluation form for your student.  This form needs to be filled in and either uploaded into your student's Google Drive Evaluation Folder or included in the portfolio which is either mailed to me or dropped off at my home.

This form is a planning checkoff course list to help the high school student plan for their high school years.  Please fill in this form for your records each year and bring it to the senior year evaluation.

If customers can’t find it, it doesn’t exist. Clearly list and describe the services you offer. Also, be sure to showcase a premium service.
